The sworn statement called for in 63-301 shall be made by filling in the Department’s standard questionnaire form and shall show:

1. (a) The experience of the applicant in handling the character of work for which it desires to become an eligible contractor.
2. (b) A description of the equipment owned or leased by the applicant.
3. (c) list of references, giving names of responsible persons having knowledge of the applicant’s character, experience and capabilities.
4. (d) Such other information as may be called for in the Department’s form.
