The factors to be considered in determining sanctions shall include, but not be limited to, the following:

1. A. Seriousness of the offense(s);
2. B. Extent of violation(s);
3. C. History of prior violation(s);
4. D. Prior imposition of sanction(s);
5. E. Provider failure to obey program rules and policies as specified in the appropriate Provider Manual or other official notices.
