| 2. | Inactive Optician: | $120 | ||
| 3. | Active Contact Lens Dispenser: | $100 | ||
| 4. | Inactive Contact Lens Dispenser: | $60 | ||
| 5. | Apprentice Renewal Fee (Annual only): | $50 | ||
| D. | Late Renewal Fee: | $25 | ||
| E. | Reinstatement Fee: | $50 |
