It shall be unlawful for any person to sell at retail any hypodermic needle or syringe unless the seller:
- (a) Identifies the purchaser by driver's license or other identification satisfactory to the seller; and
- (b) Obtains the signature, address, sex and age of the purchaser on a register which shall reflect the type and quantity of items purchased.
