1. (1) “Identifying information” means:

   1. (i) The full name of a donor;
   2. (ii) The date of birth of the donor; and
   3. (iii) The permanent and, if different, current address of the donor at the time of the donation.

2. (2) “Medical history” means information regarding any of the following:

   1. (i) Present illness of a donor;
   2. (ii) Past illness of the donor; and
   3. (iii) Social, genetic, and family history pertaining to the health of the donor.
