1. (a) General rule.— A person named in a filed certificate of authority granting that person authority may deliver to the department for filing a certificate of denial that:

   1. (1) states:

      1. (i) the name of the limited liability company;
      2. (ii) subject to section 109 (relating to name of commercial registered office provider in lieu of registered address), the address, including street and number, if any, of the registered office of the company; and
      3. (iii) the date the certificate of authority to which the certificate of denial pertains was filed; and
   2. (2) denies the grant of authority.
