1. (a) A license is not transferable.

2. (b) A license is void if one of the following conditions exist:

   1. (1) There is a change in the ownership of the legal entity, name or structure of the center, as set forth in § 11.269 (relating to change in ownership, name or structure).
   2. (2) There is a change in the location of the center.
   3. (3) There is a change in the profit or nonprofit status of the center.
