No horse shall be admitted to any part of the plant or premises of any pari-mutuel racing association under the jurisdiction of the Commission unless a health certificate, as shown in Appendix C is presented when admission to the premises is sought. The Certificate must state the following regarding the health and physical condition of the horse:

1. (1) The horse was examined thoroughly within a 7-day period preceding the date admission is sought.
2. (2) The horse was free of any evidence of infectious, contagious or transmissible disease and was afebrile at the time of the examination.
3. (3) The horse was free of ectoparasites at the time of the examination.
4. (4) The horse has not, within the past 2 weeks, been exposed to other horses with any known infectious, contagious or transmissible diseases.
