1. (a) When an individual transitions to a new provider, the current provider and new provider shall cooperate with the Department, the designated managing entity and the support coordinator, base-funding support coordinator or the targeted support manager during the transition between providers.

2. (b) The current provider shall:

   1. (1) Participate in transition planning to aid in the successful transition to the new provider.
   2. (2) Arrange for transportation of the individual to visit the new provider, if transportation is included in the service.
   3. (3) Resolve pending incidents in the Department’s information management system.
