1. (a) An individual qualified to provide behavior consultation services, mobile therapy services, behavior analytic services or behavior consultation—ABA services shall complete a discharge summary for the child, youth or young adult that includes the following:

   1. (1) Summary of the service outcomes.
   2. (2) Reason for discharge.
   3. (3) Referral for services other than IBHS if needed.

2. (b) An IBHS agency shall ensure that the discharge summary is:

   1. (1) Completed within 45 days after the date of discharge.
   2. (2) Provided to the youth, young adult or parent, legal guardian or caregiver of the child.
