QUALITY IMPROVEMENT

1. (a) A PRS agency shall establish and implement a written QI plan that meets the following requirements:

   1. (1) Provides for an annual review of the quality, timeliness and appropriateness of services, including the following:

      1. (i) Outcomes for PRS.
      2. (ii) Individual record reviews.
      3. (iii) Individual satisfaction.

      4. (iv) Number of individuals admitted to PRS that did not have a diagnosis listed in § 5230.31(a)(2) (relating to admission requirements).

         (iv.1) Average length of duration of PRS for individuals who did not have a diagnosis listed in § 5230.31(a)(2).

      5. (v) Evaluation of compliance with the approved agency service description.
   2. (2) Identifies reviewers, frequency and types of reviews and methodology for establishing sample size.
2. (b) A PRS agency shall document that individuals served participate in QI plan development and follow up.

3. (c) A PRS agency shall prepare a report that:

   1. (1) Documents analysis of the findings of the annual review required under subsection (a).
   2. (2) Identifies actions to address annual review findings.
4. (d) A PRS agency shall make the report available to the public.
