1. (a) A psychiatric outpatient clinic shall complete a discharge summary for each individual at least 45 days before discharge from services that includes the following:

   1. (1) Summary of services provided and outcomes.
   2. (2) Reason for discharge.
   3. (3) Referral or recommendation for other services if needed.

2. (b) The psychiatric outpatient clinic shall provide the following information to individuals at discharge:

   1. (1) Contact information for the local crisis intervention service.
   2. (2) Contact information for any referrals.
