Early intervention services for an infant or toddler with a disability and the infant or toddler’s family may commence before the completion of the evaluation and assessment in § 4226.61 (relating to MDE), if the following conditions are met:

1. (1) Parental consent is obtained.

2. (2) An interim IFSP is developed that includes the following:

   1. (i) The name of the service coordinator who will be responsible for implementation of the interim IFSP and coordination with other agencies and persons.
   2. (ii) The early intervention services that have been determined to be needed immediately by the infant or toddler with a disability and the infant or toddler’s family.
3. (3) The evaluation and assessment are completed within the time period specified in § 4226.61(d)(1), unless exceptional circumstances exist as set forth in § 4226.61(d)(2).
