(a) As part of the evaluation process developed by the Board and the administrator, the following must be included as a minimum:

1. (1) Of the seven patient services plus referral and information, an annual evaluation of outpatient services and at least two others, so that at the end of each 4 year period all of these services will have been evaluated once.
2. (2) A comparison of the administrator’s statistical records regarding referrals of patients to State institutions with those regarding referrals to the various services in the community.
3. (3) A review by the utilization review committee of a significant sample of the records of patients treated by the base service units and by the facilities furnishing the services being evaluated.
