1. (a) A medication log shall be kept to include the following for each child:

   1. (1) A list of prescription medications.
   2. (2) The prescribed dosage.
   3. (3) Possible side effects.
   4. (4) Contraindicated medications.
   5. (5) Specific administration instructions, if applicable.
   6. (6) The name of the prescribing physician.
2. (b) For each prescription and over-the-counter medication including insulin administered or self-administered, documentation in the log shall include the medication that was administered, dosage, date, time and the name of the person who administered or self-administered the medication.
3. (c) The information in subsection (b) shall be logged at the same time each dosage of medication is administered or self-administered.
