ELIGIBILITY REDETERMINATION

1. (a) The eligibility agency shall complete a redetermination of eligibility no less than every 12 months and establish the family’s next redetermination date.

2. (b) Prior to the redetermination, the eligibility agency shall do the following:

   1. (1) Provide the parent or caretaker with the Department’s form listing the following information last reported for each parent or caretaker or child in the family:

      1. (i) Earned income.
      2. (ii) Unearned income.
      3. (iii) Hours of work, education or training.
      4. (iv) Family composition.
      5. (v) Address.
   2. (2) Request that the parent or caretaker verify the family’s current income.

   3. (3) Verify the following factors only if the parent or caretaker reports a change:

      1. (i) Work, education or training.
      2. (ii) The number of hours of work, education or training.
      3. (iii) Family composition.
      4. (iv) Address.
3. (c) The parent’s or caretaker’s annual income must meet the requirements set forth in § 3042.31(c) (relating to financial eligibility).
