1. (a) Within 72 hours of the admission, or within 72 hours prior to the resident’s admission to the secured dementia care unit, a support plan shall be developed, implemented and documented in the resident record.
2. (b) The support plan must identify the resident’s physical, medical, social, cognitive and safety needs.
3. (c) The support plan must identify the individual responsible to address the resident’s needs.
4. (d) The support plan shall be revised at least annually and as the resident’s condition changes.
5. (e) The resident or the resident’s designated person shall be involved in the development and the revisions of the support plan.
