(b) A certificate of compliance is void without notice if one of the following conditions exist:

1. (1) There is a change in the ownership of the legal entity of the facility or agency. A transfer of stock of a corporation does not, for purposes of this chapter, constitute a change of ownership of a legal entity.
2. (2) There is a change in the name of the facility or agency.
3. (3) There is a change in the location of the facility or agency.
4. (4) There is a change in the profit or nonprofit status of the facility or agency.
