A limousine driver shall make timely written reports to the Manager of Administration as required under the act, this part or an order of the Authority, including the following:

1. (1) Invalidation of a driver’s license for any reason shall be reported within 48 hours.
2. (2) A change of address or telephone number, or both, shall be reported within 15 days.
3. (3) A change of name shall be reported to the Authority within 15 days of occurrence or if a court proceeding is required, within 15 days of the court filing.
