GROUNDS FOR TERMINATION

When at least 10 days have passed since suspension of service, the company may terminate service for failure to pay a reconnection fee and to remedy the original grounds for suspension due to any of the following reasons:

1. (1) Failure to make satisfactory arrangements to pay arrearages.
2. (2) Failure to post a deposit, furnish a third-party guarantee or otherwise establish credit.
3. (3) Failure to meet the requirements of a payment agreement.
4. (4) Failure to give adequate assurances that an unauthorized use or practice will cease.
