49 Pa. Code § 21.1207 – Conversion of licensure status related to discipline or participation in an alternative disciplinary program.

(a) Upon the effective date of a disqualifying event, defined in section 100(5) of the NLC Rules, a multistate licensee shall be automatically converted by the Board to a single-state license.
(b) At the expiration of an ordered disciplinary period or participation in the VRP, the DMU or the PERC, the licensee shall file a request to convert the single-state license back to a multistate license. A conversion fee shall not be assessed.
