1. (a) Changes that are a departure from the requirements of the program and which shall be submitted to the Board for approval include the following:

   1. (1) Changes in the objectives of the program which require alteration of the present curriculum or courses or which increase or decrease the length of the program.
   2. (2) Reorganization of the entire curriculum.
   3. (3) Changes in clinical facilities involving contractual agreements.
   4. (4) Changes in administrative control.

2. (b) When a program change is contemplated, a plan shall be presented to the Board showing:

   1. (1) Rationale for the change.
   2. (2) Present program.
   3. (3) Proposed changed program.
   4. (4) Philosophy and objectives of the proposed program.
   5. (5) Old and new master rotation or organizational curriculum plans.
   6. (6) Program bulletin and any other pertinent information.
3. (c) Fifteen copies of the materials listed in subsection (b) shall be submitted at least 3 weeks prior to the Board meetings at which the matters are to be considered.
