- (a) The physician assistant is required to notify the Board of a change in or termination of employment or a change in mailing address within 15 days. Failure to notify the Board of a change in mailing address may result in failure to receive pertinent material distributed by the Board. The physician assistant shall provide the Board with the new address of residence, address of employment and name of the registered primary supervising physician.
- (b) The primary supervising physician is required to notify the Board of a change or termination of supervision of a physician assistant within 15 days.
- (c) Failure to notify the Board of changes in employment or a termination in the physician/physician assistant relationship is a basis for disciplinary action against the primary supervising physician’s license, the primary supervising physician’s registration and the physician assistant’s license.
