4 Pa. Code § 167.95 – Certificate of education.

(a) A provider shall issue a certificate of education to a notary public applicant upon successful completion of an approved course of study.
(b) The certificate of education must consist of a certificate signed by a provider or an employee, agent, instructor, contractor or subcontractor of a provider, which contains all of the following information:
1. (1) The name of the education provider as it appears on the certificate of approval issued by the department for the approved course of study.
2. (2) The name of the approved course of study and whether it is basic education or continuing education.
3. (3) The name of the notary public applicant who completed the approved course of study.
4. (4) The date the notary public applicant completed the approved course of study.
5. (5) The statement that the certificate of education is valid for a period of 6 months from the date of issuance.
