A prospective employing law enforcement agency shall request an applicant’s separation record from the Commission when a conditional offer of employment is made to an applicant.

1. (1) A request for a separation record shall be submitted in TACS and shall include a notarized waiver form signed by the applicant.
2. (2) The Commission will provide the separation record for the applicant or certification that a separation record does not exist. If an applicant is currently employed by a law enforcement agency within the Commonwealth, the Commission will provide a prospective employing law enforcement agency with the current employing law enforcement agency information.
