   2. (2) Surcharge Remittance Form.
   3. (3) Surcharge Remittance Continuation Form.
   4. (4) Application for a Pennsylvania License to Carry Firearms.
   5. (5) Pennsylvania License to Carry Firearms.
   6. (6) Application for a Pennsylvania License to Sell Firearms.
   7. (7) Pennsylvania License to Sell Firearms.
   8. (8) Pennsylvania Sportsman’s Firearm Permit.
   9. (9) Notification of Mental Health Commitment.
   10. (10) PICS Firearm Acquisition/ License to Carry/Denial Challenge.
