34 Pa. Code § 501.2
The Office will charge the following nonrefundable fees:
| (1) | Registration | $100. |
| (2) | Biennial renewal of existing registration | $100. |
| (3) | Surcharge for late renewal of biennial registration. | $50. |
| (4) | Reactivation following suspension. | $100. |
| (5) | Registration identification card replacement. | $10. |
