1. (a) A public employe, a group of public employes or an employe organization may submit, in writing to a public employer, upon forms prepared by the Board, a notice for an election request.

2. (b) A notice for an election request shall contain the following:

   1. (1) A statement that 30% or more of the public employes in an appropriate unit desire to be exclusively represented for collective bargaining purposes by a designated representative.
   2. (2) A description of the unit deemed appropriate.
   3. (3) A request for a consent election.
