1. (a) An employer shall file a request for adjustment of a debit reserve account balance on or after the date of the Contribution Rate Notice for the calendar year immediately following the computation date corresponding to the adjustment, but not later than April 30 of that calendar year.
2. (b) The employer may not revoke the request for any cause more than 10 days after the date of filing.
