This chapter applies to the following:
- (1) An individual or group credit insurance policy.
- (2) Certificates issued under a group credit insurance policy and applications or other forms used in connection with the policy or certificate.
- (3) A premium rate charged for credit insurance.
- (4) Practices followed in providing credit insurance.
