1. (a) Except where otherwise noted in this chapter, a case shall be reported to the Department through the appropriate electronic disease surveillance system.
2. (b) A reporter may make a preliminary report of a case by telephone. The preliminary report must be followed by a formal report made through the appropriate electronic disease surveillance system.
3. (c) A case shall be reported using the appropriate case report format. The requested information shall be provided by the reporter, irrespective of the manner in which the report is submitted. Access to the appropriate electronic disease surveillance system may be obtained from the Department upon request.
