1. (a) The applicant shall be a natural person.

2. (b) An application for installer or inspector certification shall be submitted to the Department on current forms provided by the Department and must contain the following information:

   1. (1) Evidence that the applicant has the certification prerequisites contained in § 245.111 or § 245.113 (relating to certified installer experience and qualifications; and certified inspector experience and qualifications).
   2. (2) The applicant’s name, address and telephone number.
   3. (3) Other information necessary for a determination of whether the issuance of a certification conforms to the act and this chapter.
3. (c) An application for certification shall be received by the Department no later than 60 days prior to the announced date of the certification examination.
4. (d) An application must be complete upon submission.
5. (e) An applicant meeting the requirements of § 245.102(a)(4) or (b)(4) (relating to requirement for certification) will be granted admission to the certification examinations for which the applicant has requested certification and is qualified.
