A registrant or licensee shall maintain current logs, which shall be kept available for inspection by the Department for 5 years from the date of the event, showing for each radiation-producing device, the following applicable information:

1. (1) The identity (name and signature) of the operator to whom the radiation-producing device is assigned.
2. (2) The model and serial number of the radiation-producing device.
3. (3) The locations and dates of use.
4. (4) The technique factors (tube kilovoltage, tube current, exposure time) used for each radiographic exposure.
