PHASE II APPLICATION REQUIREMENTS—GENERAL

1. (a) The Phase II permit application shall comply with the following:

   1. (1) This section and § § 288.132—288.139, 288.141, 288.142, 288.151, 288.152, 288.161, 288.171, 288.181 and 288.182.
   2. (2) Chapter 287, Subchapter E (relating to bonding and insurance requirements).
2. (b) Applications, plans, cross sections, modules and narratives shall demonstrate how the construction and operating requirements of Subchapter C (relating to operating requirements) will be implemented, and shall include quality control measures necessary to ensure proper implementation.
3. (c) The plans, designs, cross sections and maps required by this section and § § 288.132—288.139, 288.141, 288.142, 288.151, 288.152, 288.161, 288.171, 288.181 and 288.182 shall be on a scale in which 1 inch equals no more than 200 feet with 10-foot maximum contour intervals.
