25 Pa. Code § 111.22 – Application.

(a) Application forms and related information concerning reimbursements may be obtained from the Department.
(b) Reimbursement requests for the preceding calendar year shall be submitted by March 31.
(c) Applications for reimbursements shall include the following information:
1. (1) An itemized statement of the following:
  1. (i) The total expenses incurred by the municipality in performing duties in compliance with the act.
  2. (ii) The total amount of fees collected by the municipality as part of its stormwater review or regulation activities in performing its duties under the act.
  3. (iii) The difference between expenses incurred and income relating to stormwater activities received by the municipality.
  4. (iv) The amount of reimbursement being requested.
2. (2) A certification as to the accuracy of the information being submitted by the chief executive officer of the municipality and the individual who prepared the statement.
(d) Municipalities shall keep accurate records and accounts of all allowable costs incurred for a period of 3 years following the submission of an application to the Department for reimbursement and shall provide access to the records and accounts to the Department and the Auditor General, upon request, for monitoring and auditing purposes.
