Probation shall terminate upon the filing of the final quarterly report and upon the expiration of the fixed period of probation, unless:

1. (a) the conditions of probation have been violated or have not been met;
2. (b) all costs of the proceedings as previously ordered by the Supreme Court or the Board have not been paid; or
3. (c) formal proceedings for discipline are pending against the respondent-attorney.
