Information as to procedure under these rules, and instructions supplementing these rules in special instances, will be furnished upon application to:

1. (1) The Office of Disciplinary Counsel, except with respect to matters which have become the subject of formal proceedings.
2. (2) The Executive Office, with respect to matters which have become the subject of formal proceedings.
