“Limited benefit coverage” means:

1. (1) Health insurance that provides:

   1. (a) Coverage for accident only, specific disease or condition only, credit or disability income;
   2. (b) Dental only coverage; or
   3. (c) Vision only coverage; and
2. (2) Independent, noncoordinated, hospital-only indemnity insurance or other fixed indemnity insurance.
