Or. Rev. Stat. § 106.320 – Form of declaration and certificate of domestic partnership

(1) The Oregon Health Authority shall prepare forms entitled:
1. (a) “Declaration of Domestic Partnership” meeting the requirements of ORS 106.325; and
2. (b) “Certificate of Registered Domestic Partnership.”
(2) The authority shall distribute the forms to each county clerk. The authority and each county clerk shall make the Declaration of Domestic Partnership forms available to the public.
