1. (1) Each Oregon certified third-party agency shall submit a monthly activity report to the Division by the tenth day of each month for the preceding month.

   1. (a) The monthly activity report shall be on Division-supplied forms or submitted by electronic mail, if available.

   2. (b) Monthly activity reports shall be completed for each manufacturer with which the third-party agency contracts and shall contain:

      1. (A) The third-party agency’s name, address, telephone number and certification number;
      2. (B) The date of the report and the reporting period;
      3. (C) The name and identification number of the manufacturer;
      4. (D) The total number of prefabricated structure inspections or plan reviews, the total number of prefabricated structures reinspected and the total number of prefabricated structures produced for siting in Oregon;
      5. (E) The total number of insignias of compliance issued by the third-party agency;
      6. (F) Signed copies of all insignia of compliance applications;
      7. (G) The number, type and date of all tests observed by the third-party agency;
      8. (H) The serial number, plan approval number, occupancy, insignia of compliance number of each prefabricated structure inspected;
      9. (I) The destination of each prefabricated structure inspected including street address and city; and
      10. (J) The signature, printed name and title of the authorized person submitting the activity report.
2. (2) Any amendments to the third-party agency’s certification approval not already reported shall be attached to the monthly activity report.
3. (3) The third-party agency shall, upon discovery, immediately submit any corrections to the monthly activity report to the Division.

4. (4) Third-party agencies shall recommend to the Division the cancellation of the Division’s registration of a prefabricated structure supplier or manufacturer when:

   1. (a) Inspections reveal the manufacturer consistently fails to comply with approved plans or the Oregon Specialty Codes;
   2. (b) A manufacturer relocates or makes changes to the quality assurance program without giving prior notice to the third-party and the Division; or

   3. (c) A manufacturer builds prefabricated structures without first obtaining approved plans.
