1. (1) License renewals must be completed on or prior to the license expiration date by:

   1. (a) Submitting a renewal application or completing the online renewal form;
   2. (b) Completing all continuing education requirements; and
   3. (c) Paying the license renewal fee .
2. (2) The division mails one renewal notification to the last known address of the licensee at least 30 days prior to license expiration. It is the responsibility of the licensee to notify the division of a change in the licensee's address.
