1. (1) A change of physical address of the PPK requires:

   1. (a) Submission of a new PPK application a minimum of 15 days prior to occurrence;
   2. (b) Registration fee;
   3. (c) Approval of the board; and
   4. (d) Issuance of a new certificate of registration.

2. (2) A change of location at the same physical address of the PPK requires the submission of an updated floor plan drawn to scale a minimum of 15 days prior to the change with the new location of the:

   1. (a) PPK within the building;
   2. (b) Surveillance system cameras; and
   3. (c) Alarm system panel.
