1. (1) Except as defined in SB 450 (2023), the label on a drug dispensed or distributed from a charitable pharmacy must meet all federal rules and laws and must contain:

   1. (a) The name, address and telephone number of the pharmacy;
   2. (b) The name of the prescribing practitioner;
   3. (c) The initials of the dispensing practitioner;
   4. (d) Date dispensed;
   5. (e) The name of the patient;
   6. (f) Name and manufacturer of drug, drug strength, the quantity dispensed;
   7. (g) Directions for use;
   8. (h) The expiration date;
   9. (i) A unique identifier; and
   10. (j) Any further cautionary information required for patient safety.
2. (2) All original patient identification must be removed.
