Or. Admin. R. 851-031-0086 – Retired Status

(1) An individual may retire their LPN, RN, or APRN license by submitting a completed application, which includes:
1. (a) The Board’s retired status application form;
2. (b) Confirmation the license is in good standing with the issuing authority and not subject to any disciplinary investigation or action pending; and
3. (c) Surrendering the license being retired.
(2) A license in retired status does not authorize practice under the license.
(3) Incomplete applications expire 12 months from the initial submission date.
