Or. Admin. R. 848-040-0160 – Standards For The Documentation of a Reexamination

(1) When a physical therapist is required to perform a reexamination under OAR 848-040-0155, the documentation of the reexamination shall include at a minimum:
1. (a) Subjective status of patient; and,
2. (b) Any change in physical therapy goals (including patient goals) or the plan of care.
(2) After a physical therapist reexamines the patient and documents the reexamination, either the physical therapist or a physical therapist assistant may prepare the documentation of the patient’s current status.
