License Surrender

  1. (1) To surrender a license issued under ORS Chapter 471:

    1. (a) A licensee must submit a written request to the Commission on a form and in a manner provided by the Commission; and
    2. (b) The Commission must approve the request.
  2. (2) The surrender of the license is effective on the date the Commission approves it.
  3. (3) The effective date of the license surrender may be no earlier than the date the Commission receives it.
