1. (1) Any covered employer, owner or operator may apply for an exemption from all or part of the reporting requirements of the Community Right-to-Know and Protection Act and its administrative rules.

2. (2) An application for an exemption must be in writing and contain the following:

   1. (a) The name and mailing address of the person making application;
   2. (b) The site address and location of the facility;
   3. (c) The facility number assigned by the State Fire Marshal;
   4. (d) The rule or law, identified by number, from which the exemption is sought; and
   5. (e) The basis for the request.
