1. (1) A carrier or third party administrator must do all of the following:

   1. (a) Allow enrollees to submit the standardized form entitled “Oregon Confidential Communication Request” set forth on the Division of Financial Regulation website of the Department of Consumer and Business Services at dfr.oregon.gov.
   2. (b) Acknowledge receipt of the enrollee’s form and respond to an enrollee’s confidential communications request.
   3. (c) Include with the acknowledgement any information the enrollee needs about the effect of the request and the process for changing the status of the request.
2. (2) A carrier or third party administrator must communicate with providers about the protections afforded to enrollees under chapter 470, Oregon Laws 2015 and at a minimum provide information about how to access the “Oregon Confidential Communication Request Form.” A carrier or third party administrator is not limited to providing information about the form.
