1. (1) A legal service organization shall amend its registration in this state when any of the following events occur:

   1. (a) The organization changes its principal place of business;
   2. (b) The insurance or surety bond required for compliance with ORS 750.685 is cancelled or replaced;
   3. (c) The organization experiences a material change in ownership under events defined in ORS 732.505 and 732.510.

2. (2) When a legal expense organization changes its principal place of business, the organization shall include the following in the amended registration:

   1. (a) The street address, including city and state;
   2. (b) The mailing address, if different;
   3. (c) The telephone number.
3. (3) If an organization changes its registered office or agent, the organization must file a statement of the change with the Director on the form prescribed by the Director.
