1. (1) Duplicate licenses or certificates may be obtained by:

   1. (a) Certifying, by signed statement, that the current license or certificate has been lost or destroyed; or
   2. (b) Requesting a duplicate for additional place(s) of business;
   3. (c) In either case, payment of the required fee.
2. (2) Reproduction of a license or certificate by anyone other than Board staff for use as a license document or for display is prohibited.
